Community Development Planner

Location: Owensboro, KY

The Green River Area Development District (GRADD) is seeking a highly motivated Community Development Planner to join our Community & Economic Development department. This role offers a unique opportunity to support the development and administration of projects that improve the lives of residents across our seven-county region in western Kentucky.

About the Role

The Community Development Planner provides direct assistance to local governments, helping them plan for the future while managing current development efforts. This position works closely with elected officials and state and federal partners to support funding, compliance, and planning activities that impact economic development, infrastructure, housing, hazard mitigation, and more.

Key Responsibilities

  • Assist in developing and administering community development projects through partnerships with agencies such as the Economic Development Administration, USDA-Rural Development, EPA, and others.
  • Provide grant writing and technical assistance to cities, counties, and local agencies for programs including CDBG, Homeland Security, E911, and infrastructure improvements.
  • Research, analyze data, and contribute to long-range strategic and regional planning efforts such as water and wastewater management, environmental protection, and economic development.
  • Respond to information requests from local government partners and identify new funding opportunities aligned with GRADD’s mission.

Qualifications

  • Bachelor’s degree in planning, public administration, political science, regional & urban planning, or a related field.
  • Strong oral and written communication skills.
  • Detail-oriented with excellent organizational and multitasking skills.
  • Team player with a collaborative approach to problem-solving.
  • Ability to work with government agencies, local officials, and the public.

Case Manager (Hybrid/Work from Home Option)

Job Description:

Green River Area Development District (GRADD) is seeking a dedicated Case Manager to provide support and coordination for individuals with intellectual or developmental disabilities through Kentucky’s Home and Community-Based (HCB) Waiver program. This role ensures clients receive the necessary resources to live as independently as possible within their communities.

The HCB Waiver is part of Kentucky’s 1915(c) HCBS Medicaid waiver program, allowing participants to hire their own employees for non-medical, non-residential services under Participant Directed Services (PDS).

Key Responsibilities:

  • Perform case management functions for the Home and Community-Based Waiver (HCB) program.
  • Review client eligibility and coordinate reassessments to maintain Medicaid Waiver compliance.
  • Complete and submit all required Medicaid waiver forms using the Medicaid Waiver Management Application (MWMA).
  • Conduct initial and annual reassessments, including recertification and care plan development.
  • Maintain Participant Directed Services (PDS) employee and participant documentation per agency guidelines.
  • Assist PDS employees and participants with necessary paperwork and guidance.
  • Monitor member budgets, health, safety, and welfare through timesheet submission and compliance checks.
  • Advocate for clients and families by connecting them with appropriate community resources.
  • Conduct monthly home visits to monitor services and track progress toward client goals.
  • Collaborate with GRADD’s PDS Manager, provider agencies, Department for Community Based Services, and Department for Medicaid Services to meet client needs.
  • Maintain accurate participant records, logs, and reports.
  • Refer waiver applicants to the Aging and Disability Resource Center staff in a timely manner.
  • Attend staff meetings and professional development trainings.

Other Responsibilities:

  • Assist with special projects as needed.

Qualifications & Requirements:

  • Education:
    • Bachelor’s degree in Social Work, Gerontology, Psychology, Sociology, or a related field with two (2) years of experience working with individuals with special needs.
    • Bachelor’s degree in Nursing with a current Kentucky nursing license and two (2) years of experience working with special needs populations.
    • Associate degree with two (2) years of experience working with special needs populations.
  • Experience:
    • Reliable transportation required.
    • Knowledge of special needs populations, community organizations, and service resources.
  • Skills:
    • Strong verbal and written communication skills.
    • Excellent organizational skills and the ability to multitask.
    • Proficiency in computer applications and documentation software.
